We offer outpatient physical, occupational, and speech therapy to members of the Bracken County community — the same leading-edge equipment and experienced therapists that power our inpatient programs, on a schedule that fits your life.

How to get started — step by step
1. Get a referral
Ask your physician, surgeon, or hospital care team for a referral to Perkins Country Manor outpatient therapy — they can send your order directly to us. Not sure whether you need a referral? Call us at (606) 756-2156 and we’ll walk you through it.
2. Schedule your evaluation
Call us at (606) 756-2156 to schedule your first visit. Our team will verify your insurance coverage and find an appointment time that works for you.
3. What to bring
For your first appointment, please bring a photo ID, your insurance card, your physician’s referral or order, a list of your current medications, and comfortable clothing and supportive shoes you can move in.
4. Where to check in
Enter through the main entrance at 5269 Asbury Rd, Augusta, KY 41002 and check in at the front desk — a team member will show you to the therapy gym.
5. Outpatient hours
Outpatient therapy is offered by appointment, Monday through Friday.
